Your First Visit

For your convenience, we have provided our patient forms online below. Please print out the forms, complete them and bring them with you to your appointment. Please bring all your dental insurance information, medical information, and emergency contact information when you come.

An adult new patient preventive appointment typically requires approximately one and one-half hour. This appointment usually includes a full mouth cleaning, comprehensive exam, any necessary X-rays, an oral cancer exam, and a full-mouth periodontal evaluation. Personalized oral hygiene instructions will be provided along with suggestions to help you care for your particular oral situations.

For persons ages fourteen to sixteen, an initial preventive appointment requires approximately one hour. Typically, the same services as above are offered, except for the periodontal evaluation.

For persons under fourteen years of age, an initial preventive appointment requires approximately forty minutes. This appointment includes a full mouth cleaning, necessary X-rays, comprehensive exam, and a fluoride treatment. Periodontal evaluations are usually not necessary on children.

Of course, each patient presents different situations and needs; therefore, the first visit can vary due to special circumstances.
